Rapid Re-Housing & Transitional Housing
The Eastern Regional Family Resource Network offers critical support to young adults facing housing instability through its Home 2 Maple Hill Rapid Re-Housing and Transition House programs. These initiatives aim to provide stable housing and supportive services to young adults between the ages of 18 and 24 who are experiencing homelessness or are at imminent risk of homelessness.
PROGRAM OVERVIEW​
1. Rapid Re-Housing

Objective: To quickly transition young adults from homelessness into stable housing.

Services Provided: Short-term rental assistance, case management, and linkage to supportive services.

2. Transition House (Home 2 Maple Hill)

Objective: To provide a safe and stable living environment while young adults work towards independence.

Services Provided: Housing, case management, life skills training, and access to educational, employment, and other resources.
ELIGIBILITY REQUIREMENTS
To qualify for either the Rapid ReHousing or Transition House programs, applicants must meet the following criteria:
Age Requirement

Age Range: Applicants must be between 18 and 24 years old at the time of application.
Housing Status

Literally Homeless: Applicants must lack a fixed, regular, and adequate nighttime residence. This includes:

Living in places not meant for human habitation, such as cars, parks, sidewalks, or abandoned buildings.

Staying in emergency shelters or transitional housing for homeless individuals.

On the Verge of Homelessness: Applicants facing imminent risk of losing their primary nighttime residence. This includes:

Receiving an eviction notice from a landlord.

Being asked to leave by friends or family with whom they have been staying temporarily.

Experiencing Homelessness from Domestic Violence: Applicants fleeing or attempting to flee domestic violence, dating violence, sexual assault, stalking, or other dangerous or life-threatening conditions related to violence.

APPLICATION PROCESS​

1. Initial Screening: Prospective applicants undergo an initial screening to determine eligibility based on age and housing status.

2. Intake Interview: Eligible applicants participate in an intake interview to assess their specific needs and the appropriateness of program services.

3. Documentation: Applicants must provide documentation to verify their age and housing status, including identification, eviction notices, or evidence of homelessness or domestic violence.

4. Case Management Assessment: A case manager conducts a comprehensive assessment to develop an individualized housing stability plan.

SUPPORTIVE SERVICES
Both programs offer a range of supportive services to assist participants in achieving and maintaining housing stability:
